Crafts Zone Summer Program
Disclosure and Agreement
By submitting the registration form for the Crafts Zone Summer Program, I, as the parent or legal guardian of the participant, acknowledge and agree to the following terms:
1. Assumption of Risk & Release of Liability
I understand that participation in the Crafts Zone Summer Program involves various activities, including the use of art and craft materials, which may pose risks of injury or property damage. I voluntarily assume all risks associated with my child’s participation and release Crafts Zone Studio, its owners, employees, and representatives from any claims, liabilities, damages, or losses arising from participation in the program.
2. Medical Authorization
In the event of a medical emergency, I authorize Crafts Zone staff to seek appropriate medical treatment for my child. I understand that all reasonable efforts will be made to contact me or the emergency contact listed on the registration form before treatment is administered.
3. Photography & Media Release
I grant permission for Crafts Zone Studio to take photographs or videos of my child during the program for promotional, marketing, or educational purposes. I understand that these images may be used in online and print materials without further consent or compensation.
4. Behavioral Expectations
I understand that my child is expected to follow all program rules and behave in a respectful and cooperative manner. I acknowledge that inappropriate behavior may result in my child’s removal from the program without a refund.
5. Agreement to Terms
By submitting the Crafts Zone Summer Program registration form, I confirm that I have read, understood, and agreed to all the terms outlined above. I acknowledge that this agreement is legally binding and applies to my child’s participation in the program.
